Basic and Optional Life*

  • Provider: Minnesota Life

Basic Life and Accidental Death and Dismemberment

1x annual adjusted base salary given by Miami-Dade County Office of the Tax Collector.

  • No cost, but employee must be actively at work for coverage to start.
  • Cost of coverage above $50,000 is taxable to the employee.

Optional Life: Employee Life

Available in increments of 1x–8x annual adjusted base salary; maximum $2,000,000.

  • Evidence of Insurability (EOI) required if electing more than 3x salary during initial New Hire Enrollment or when increasing coverage during Open Enrollment.
  • Spouse/Domestic Partner & Child Life: $10,000 available for each.
  • Employee must be enrolled in Employee Life to elect Spouse/DP or Child Life. EOI is never required for either.
